All the incidents are caused by :-


a)Unsafe act.
b)Unsafe condition.

1. Not wearing PPE,
2. Using uncertified Lifting equipment,
3. Driving without License, Not wearing Seatbelt while travelling,
4. Working at height without fall protection,
5. Using improper tools,
6. Working or walking under suspended load,
7. Loading or unloading long objects without Tag line,
8. Working in the electrical equipment without isolating power,
9. Changing Tong Dies without isolating Hydraulic pressure,
10.Using mushroomed tools, etc

1.Not following the procedure,
2.Over confidence,

5.Not paying proper attention to the Supervisors,
