What is Scaffold?
A scaffold is any temporary elevated platform (supported or
suspended) and its supporting structure (including points of
anchorage), used for supporting both employees and the
materials to be used
How do you prevent falls from scaffolds?
Implement a comprehensive safety program.
Understand KOC HSEMS procedures- KOC SA. 002 scaffold
procedures.
Train workers to identify hazards associated with scaffold use.
Use safe work practices.
Scaffold Inspector
A competent person (3rd party certified) whose combination of
knowledge, training and experience is appropriate for the type and
complexity of the scaffold.
A non-scaffolder who has attended a scaffold inspection course (eg a
site manager) could be deemed competent to inspect a basic scaffold
structure.

Scaffold inspection report requirement
Report includes:
Name & location of the person on whose behalf the inspection was carried
out
Location of the scaffold inspected
Date & Time of inspection
Details of any faults identified
Any actions taken to rectify those faults
Any further actions required
Name & position of person making the report.
Repair
The scaffold competent person must be notified immediately if a
component becomes damaged after erection, and the scaffold must
RED TAG and not be used until the damaged component has been
properly repaired or replaced.
Scaffolds General Requirement
Scaffolds shall be erected, modified or dismantled only by 3rd party
certified scaffolding personnel's.
Scaffolds shall be constructed in compliance with KOC Recommended
Practice for Scaffolding (KOC-L-025).
Scaffolds shall be inspected on the following occasions:
- before any scaffold is used
- following any alterations
- after any period of adverse weather
- every 7 days, whether scaffold was used or not
- at regular intervals during use in high wind conditions.
Completion of scaffold inspection register.

At start of each shift or after modification by competent person
Do visual inspection for obvious issues:
- Access
- Full Planking
- Guardrails
- plumbness
Competent Person Inspection Checklist
When was the last time scaffold was inspected
Were the planks looked at ( checked for saw cuts, fungi, dents , face
breaks, splits, corrosion, bended, cracks, etc.)
Is there any ladder? ( Obstruction access, three feet above, with in two
feet of the bottom, built in)
Who trained the erecting and dismantling crew?
Last time the users trained?
What is the maximum intended load of the scaffold?
What is the maximum load intended to put on scaffold?
Competent Person Inspection Checklist
Plumbness
Base plates, mudsills? ( What is the foundation likes)
Working level fully planked?( holes, max. 9 ½.)
Extension of planks over the end frame? (6 – 12.)
Fall protection? (When & What)
Securing the scaffold at 4:1? (width of frame)
Maintained clearance near ALL power lines
Scaffolding must be fully planked
Legal Requirements
Inspections are required on all scaffolding from which a person can
fall
Before use
After substantial alteration
After adverse weather or occurrence
Within every seven (07) days
All scaffolds shall be in accordance with KOC Recommended Practice for
Scaffolding (KOC-L-025) : KOC.SA.002 - Control and Use of Scaffolding
What is Scafftag?
SCAFFTAG
A proprietary system for recording scaffold inspections comprising a
holder (which, when empty, denotes that the scaffold should not be
used), and an ‘Erection and Inspection Record Card. Scaffold approved for
use will have a completed

Date erected, with name and initials of Scaffolder;
Maximum loading kN/m2;
Date inspected, with name and initials of Scaffolding
Contractor’s Representative;
Date modified, with name and initials of Scaffolder;
Date re-inspected, with name and initials of Scaffolding
Contractor’s Representative;
Expire date of label;
Dismantling date.
A Scaff specific card holder and card, that when fixed to a scaffold,
identifies the status of the scaffold, details with respect to its use and
inspection detail

Note:
-Scafftag are to be signed / inspected either
by the scaffolding supervisor / inspector / or
a competent person .
-Scaffold must be checked every 1 week (7
days), then updated the Scaff tag as
possible.

SCAFFTAG displayed at each access point. This will show the signed-off
green inspection card, which has provision on the reverse for re-
inspection records.

Independent scaffold
Independent Scaffolds- these scaffolds which have two rows of standards each rows
joined together with ledgers which in turn support the transverse transom. This scaffold is
erected clear of existing or proposed building but is tied to the building or structure at
suitable intervals.
Birdcage
A birdcage scaffold consists of a mass of standards arranged at regular intervals in
parallel lines, usually evenly spaced apart. These standards are laced together with a grid
of ledgers and transoms at every lift height. The top lift is boarded to form the access
platform for work on ceilings and soffits, e.g. fix lighting, ventilation or sprinklers over an
inside area.

Putlog Scaffolds- these are scaffolds which have an outer row of standards joined
together by ledgers which support the transverse putlogs which are built into the bed
joints or perpends as the work proceeds, they are therefore only for new work in bricks
or blocks

Bridle Tube-
Where it is necessary by-pass a window or door
opening a bride tube is connected to the underside
of the putlog and act as a support for the end of
putlog not supported by the brickworks. It should
be fixed with right angle couplers.
The bride tube also serves as the inner fixing point
for braces.
