  • Physiotherapy combined with dry needling among patients with chronic low back pain: Study protocol for a randomized controlled clinical trial

    ELSEVIER

Background Chronic low back pain (CLBP) is an extremely common public health concern responsible for pain-related disability. CLBP is challenging to manage despite having a plethora of treatment options. Physiotherapy is a guideline-recommended treatment for CLBP. Furthermore, some forms of complementary medicines, such as dry needling, spinal manipulation, Tai Chi, and yoga are also recommended for CLBP treatment. We hypothesized that the combined treatment would be more effective when managing CLBP. Therefore, this randomized clinical trial aims to examine the impact of combined therapy of dry needling and physiotherapy compared to the treatment effect of only physiotherapy among patients with CLBP. Methods The study is a two-armed single-center, randomized controlled clinical superiority trial where participants are randomized to combined therapy of usual care physiotherapy and dry needling or only usual care physiotherapy (1:1). Individuals who are 18 years or older and experiencing LBP with or without leg pain for a minimum of three months will be considered eligible for the study. Pain severity, pain affective and physical interference, activity limitation, and insomnia symptoms of patients with CLBP will be measured at the baseline after four, 12 and 24-week treatment started. Conclusion Finding a better management strategy for managing CLBP is an ongoing challenge. Most of the novel techniques that try to manage CLBP are limitedly tested. This study will allow testing of the combined effect of usual care physiotherapy and dry needling when managing CLBP in terms of clinical efficacy. If the combined therapy is proven significantly effective, compared to usual care, physiotherapy alone will provide plausible evidence of an effective treatment option to manage CLBP. Background: Osteoarthritis (OA) a common disease of aged population and one of the leading causes of disability. Incidence of knee OA is rising by increasing average age of general population. Age, weight, trauma to joint due to repetiting movements in particular squatting and kneeling are common risk factors of knee OA. Objective: This descriptive cross sectional study was conducted to assess the pattern of osteoarthritis among patients attending selected specialized physiotherapy hospital. Methods: A pre-tested, modified, semi-structure questionnaire was used to collect the data with a sample size was 100. Data were entered and analyzed by using SPSS software. Verbal consent was taken before initiation of interview. Results: Most of the patients (40%) were suffering from osteoarthritis in 50-59 years age group. About 22% patients were from 60-69 years age group. Housewife, service holder and businessman were 36%, 28% and 16% respectively. Knee was the most affected (79%) organ of osteoarthritis followed by hip (17%) and both knee & hip (4%). Those who did moderate type of activity, 63% of them suffered from osteoarthritis followed by sedentary activity (32%) and hard worker (5%). Obese and overweight patients were 55% and 39%. Conclusion: Age, occupation and physical activity level may be determinant of osteoarthritis and study result should not be generalized because of small sample size. Further large scale study may be instituted to get actual picture and precise result.

For different population World Health Organization and other international health organization has to take different initiatives. For the first world they just play the role of supervisor and advocate. On the other hand for the third world countries they have to collect funds as well help in policy making. Another important perspective of modern health care system is the health care for the minorities. Providing equal health care opportunities for the minorities is another challenge of this new century. Also in the several part of the world where people are tearing apart each other by war and violence to maintain health care is not only a challenge but also struggle for the health care providers. Type-2 diabetes is now preventable. The prevention of diabetes by life style modification, including adequate exercise is more effective in preventive diabetes than pharmacological therapy. Most people with diabetes suffer from musculoskeletal complications which might include frozen shoulder, Back pain or osteoarthritis and many people with Type-1 diabetes develop a syndrome of limited joint mobility. Physiotherapy is a modern medical science which involves physical treatment techniques, such as exercise, massage and the use of electrotherapeutic and mechanical agents rather than drug therapy for the management of a condition. Physiotherapy has got important role not only in the management of disabilities but also in the management of diabetes, for control of sugar level in euglycemic state. Exercise has got tremendous role in regulating the diabetes. It lowers blood sugar level, decrease excessive body fat, decrease triglyceride, increased high density lipoprotein, decrease atherosclerosis, decrease cardiovascular and peripheral vascular disease and also increase sensitivity of insulin. Best exercise advised or given by a physiotherapist when a diabetic patient is disabled. So-that physiotherapy has a vital role in the
