Bony Nose VS Fat Nose Surgery

Bony Nose VS Fat Nose (Fleshy Nose): The main difference between fat nose and bony skin is nasal skin thickness. Meaty noses are more form-able and bony noses are easier to get smaller.

Some people have thin-skinned bony noses and are dissatisfied with the shape of their noses; On the other hand, some others have fleshy (fat nose) noses with a big part of cartilage; all of them think, form, and size of their noses do not fit their face, and that’s why they want to do nose surgery. Let’s talk about these kinds of noses, characteristics, and nose job in Iran.

Types of noses

You have heard a lot about types of noses, probably. Some people categorize noses based on ethnicity and race, and others classify them as fat noses vs bony noses. These dissatisfied people come to plastic surgeons to change their facial expressions; a nose job is one of the most common cosmetic surgeries, like in many other places. Iranian men and women desire to have European noses, which they think are the best facial profile.

Bony versus meaty nose (Fat nose) surgeries

As mentioned before, people generally divide their noses into two meaty and bony groups; this classification is not scientifically based, but surgeons use these terms to better communicate with patients; the significant difference between them is nasal skin thickness. While a thick skin covers fat noses, bony noses have thinner skin and a fat layer. Fat noses are more form-able, while bony ones are easier to make smaller and shaped. In nose surgery, both of these features are needed. The surgeon should be able to shrink around the nose, a more straightforward task, and form the nose tip, a more difficult job to do. 

During the surgery, the deviation and curvature of the septum (cartilage dividing nostrils into two parts) are fixed. To fix the nose’s high bump, a skilful surgeon ensures the surgery won’t cause a problem for the patient’s breathing. Meaty/fleshy noses (Fat Nose)

Practically, it is more challenging to get a good result from a fleshy nose surgery than from a bony nose, and it needs a very experienced surgeon to do that; look at these two samples here:

Unlike their names, fat noses do not have a lot of meat or muscle in them. But they have:

  • Thick and oily skin with noticeable pores (holes).
  • Wide fat nose tip
  • Thick nose blades
  • The wider lower part of the nose
  • Weaker nasal cartilage
  • High nose blade height from aside

These specifications make the nose broad and bulky; they also blur nose lines, forming a more beautiful nose.

Fleshy nose (African) types and their surgery

let’s talk about African nose types here:

A fat nose with large pores and hyperactive fat glands

There are some fat noses with large pores and fat glands under the skin; these skins are called oily. These patients’ skin exfoliation using a laser or a low-salt diet is recommended. The surgeon usually suggests waiting until six months after the treatment for patients under fat control treatments.

Some other patients with fat noses and underactive glands have red noses. In surgery of this kind of nose, the surgeon creates a more beautiful form with a bony and firm skeleton under the thick nose skin; sometimes, the surgeon brings nasal skin and cartilage closer together to prevent excess tissue and discharges in the nose tip.

Old surgery methods for fleshy noses were not helpful because, after a while, the shaped cartilage would turn back to its former shape. In fat nose surgery, keeping facial fitness is more vital; if the nose becomes tiny or its curvatures increase significantly, it could cause breathing issues.

As mentioned before, African-type or fleshy nose skins are thick and have fat glands and weak cartilage. This fragile structure makes meaty nose surgeries more sensitive. That’s why these patients are asked to wear strips on their noses for a more extended period after the surgery than bony structure noses. Fleshy/fat nose surgery is like a double-edged sword. On the one hand, this large and thick skin covers nose structure defects; on the other hand, its recovery takes much longer. Techniques for fat nose surgery

  • Fat reduction in soft tissue: soft tissue-forming fats, especially in nose tips, are skillfully thinned to show nose properties better.
  • Proportional reduction in nose size: If the distance between the skin and nose skeleton is too much, there’s a higher probability of scarring or extra fat creation, which causes unfavorable nose deformation.
  • Structural nose support: thicker skin requires a more robust structure due to its heavier weight. So, experienced Iranian cosmetic surgeons will create a correct proportion between nose size and structure. After returning the nose skin to its natural position, the reinforced structure helps the skin to position better.

Generally, meaty, thick-skinned nose surgery needs high expertise and experience, as the surgeon’s proper perception of suitable nose size to get a good result is vital.

Fat nose surgery is more complicated.

Most surgeons’ efforts in African nose Rhinoplasty are devoted to reforming nose cartilage and bone to shape nose structure. On the other hand, the quality and thickness of skin are determinant factors of surgery outcome; that’s why the final result of surgery could be seen after the contraction of soft skin tissue toward the nose structure, which could take months.

It would be best to wait even more weeks or even months after African nose surgery to see the final result of the operation after the complete disappearance of swelling and bruising of surgery.

Do African / fat noses get back to their prior form after a nose job?

After the surgery, swelling and bruises in fleshy noses take longer to fade than in bony noses, but contrary to the general belief, fleshy noses do not return to the first condition after the surgery. The success rate of surgery for fat noses is higher in women than men.

Generally, patients with fat noses should have more realistic expectations of their nose surgery because it is unlikely to have the same result as bony nose surgeries.

In pre-operative sessions, surgeons tell patients it is better to expect a 40-70% improvement in their nose surgery, while people with a bony nose will even see the best results.

Fat nose surgery after-care

You have to follow your surgeon’s instructions after the surgery strictly:

  • Using a cold compress to reduce swelling and bruising
  • Timely use of prescribed drugs
  • Use the prescribed cold medicines to prevent cold and cough

To prevent forming a new skin to get a better result, in the long run, keeping the nasal strip for at least two months on the nose is usually necessary. For some patients, subcutaneously corticosteroid injections are prescribed for the upper part of the nose tip to reduce skin thickness; this injection, done by a skilled surgeon, may need to be repeated several times.

What about bony noses?

Bony or humpy noses have thin skin; this thin skin helps the surgeon achieve a well-shaped nasal bone and makes surgery more wonderful. In summary, bony noses have three main specifications:

  • A hump on their bridge
  • Thin high flexible skin
  • Strong tissues and cartilage

The nose structure has three primary layers. First, A delicate muscular layer that attaches to the bottom part. The second is a bony-cartilaginous layer, and the third is a layer covering mucous in the internal part of the nose.

As mentioned earlier, plastic surgeons divide noses into two types, meaty and bony noses, according to the type of tissue and cartilage; the difference is in their tissue and cartilage. Cartilage is more resistant to thin/bony noses and weaker in meaty ones. Bony noses often have more curvature and hump in their bridges.

Awkward, excessive growth of nasal cartilage could be due to hereditary factors or bone fractures caused by accidents. This in-consistence growth of nasal cartilage is the cause of bony nose formation. Sometimes, the nose’s shape does not change much due to childhood injuries, but after the child grows, the nose form changes. The most major task of surgeons in bony nose reshaping is hump removal.
