Max is drawing Koko and Fitz when his chair leg breaks. This sends him tumbling and draws an odd shape in Koko's neck. He goes to see a chiropracter. This inspires him to go into that line of work. Meanwhile, a bone crusher is working on Max, and lifting his tie pin and wallet.
As above, so below, and chiropracty was becoming a well-known way of treating issues in the 1920s. Even Harold Lloyd had made a short subject on his trying to 'encourage' people to need some work done, just as Fitz does here for the clown. Unfortunately for them, where the cartoon laws of physics apply, even a banana pee on the ground won't get you a tumble.
