If the child has no legal guardian, or their guardian is declared negligent or incompetent, then a judge can appoint a guardian ad litem for the child, which is a type of temporary legal guardian who can make medical decisions that are in the child's best interest.

When responding to the kid stuck in an airplane after his father had a heart attack Roy and Johnny realize it would help to have a pilot present to talk the kid through landing the plane; but there were no pilots at the airport at the moment and they act like they don't know where to find one on short notice. However the Los Angeles County Fire Department, Los Angeles County Sheriff's Office and California Highway Patrol all have pilots and there should always be at least a few on duty at any given time. Between those three organizations it should be easy enough to get a pilot down there quickly, or at least get one on the radio for the boy to talk to.
