When Teresa catches Jordan in his affair, the limo disappears, then reappears, driving away, as she beats on him outside the hotel.
When Jordan is explaining the origin of Quaaludes around the pool table, Donnie knocks a beer off a ledge. In the next shot the beer has returned, only to disappear in the third shot.
When Jordan asks Brad to sell him the pen at the diner, in one shot Brad grabs the pen, but in the next shot he grabs the pen again.
When the Naomi is going through "chop," Donny's wife is no longer present on the yacht with the others, despite the rest of the boat being flooded.
When Teresa discovers Jordan's affair with Naomi, after he gets out of the limousine, she slams the car's door twice.

Factual errors

Belfort says that he takes Adderall to stay focused. Adderall was not released until 1996 and was not released as a generic until 2002. Given that the film spans from 1987 to 1999, using "prescription amphetamine pills to focus" would be much more accurate.
Towards the end of the film, Jordan puts his daughter in a late model (either a 1999 or a 2000) R129 Mercedes SL500 to take her away from Naomi. Not only does this car not fit the time frame of this film, but the way he engages the parking brake in reverse (with his right hand pulling upwards) is incorrect on this type of vehicle. The parking brake is located near the brake pedal and must be pressed down with your left foot.
According to Belfort, he did not back out of his deal with the SEC, and his "they will have to call in the National Guard" speech never happened.
Pulling back on the cyclic pitch lever, in front of the pilot, of a helicopter will not arrest a rapid descent. That is the job of the collective lever, which is found near the pilot's thigh.
Rao's is so exclusive, table assignments are made in increments to those the owner knows personally. The only way an outsider such as Belfort and Dietl can dine there is if they are invited to by someone who "owns" a table.

Incorrectly regarded as goofs

When Jordan is leaving the country club and crawls to the steps, the number of steps changes when viewed from Jordan's perspective and also when he rolls down them. This isn't a continuity error. Because Jordan was high on drugs his perspective of reality was affected, so to him there were more steps to navigate. Just as he later claimed to have driven home without scratching the car, when he had actually trashed it. His perspective of reality was affected by the drugs.
According to Belfort, there was no midget tosses during his tenure. However, Danny Porush said he and others talked about holding midget tosses.

However, this cannot be considered a factual mistake but is rather a dramatization choice or creative liberty taken by the writer(s) and/or director.
When Jordan is having a flashback to driving his Lamborghini out of the country club while high on Lemmons, we see him yank the steering wheel to the left; but in the following shot, the car moves forward and turns to the right. However, this is not a goof since Martin Scorsese has confirmed that where Jordan has taken drugs the editing becomes odd on purpose, with continuity issues to make events flow oddly.
According to Belfort, the yacht did not turn on her side during the storm nor did the helicopter fall off on its own. The authorities ordered the crew to push the helicopter off the yacht so that the Italian special forces could land on the helipad to rescue all on board.

However, this cannot be considered a factual mistake but is rather a dramatization choice or creative liberty taken by the writer(s) and/or director.
Belfort's private jet did not fly to the scene of the boating accident. According to Belfort, after the rescue, he learned the jet had crashed after takeoff. Fortunately, the pilot, the only person on board, survived.

Revealing mistakes

During the Black Monday crash of 1987 all of the digital tickers in the background are green - which means the market is going up.
The face of Leonardo DiCaprio's stunt double is visible after Jordan rolls down the stairs.
When Donnie urinates on the subpoena in the trash can, there is a clear line in which the urine stream simply disappears slightly above the can. This reveals that it was an effect added in editing.
When Jordon and Naomi have sex for the last time, the special sock Leonardo had to wear on his genitals is visible.
In the flashback of Jordan leaving the country club while under the influence, he says he's driving a Ferrari. However, he is actually driving a Lamborghini Countach.

Miscellaneous

In the photo of Jordan and his father at Fenway park at 1:48, the photo is supposed to be set in the late 70s/ early 80s, but in the background, there are seats on top of the "Green Monster" (left field wall), which were not added until 2003.

Anachronisms

Stocks are being traded in dollars and cents (5.25 and 3.19). At the time this story took place, stocks were still being traded in dollars and fractions (5 1/4 and 3 3/16).
At the Stratton Oakmont trading floor, an electronic ticker shows the symbol "HPQ," Hewlett-Packard's symbol after purchasing Compaq. HP bought Compaq in 2002.
Belfort claims to use Adderall as one of his many drugs of choice, however the drug was not on the market until 1996, four years after Belfort was indicted by the SEC.
The Long Island strip mall features a Radio Shack with its 1995 logo.
The scene in Geneva shows a modern tram built after 2000.

Audio/visual unsynchronised

When Donnie and Jordan meet for the first time in the diner, Jordan says "Well technically, 72,000, last month, something like that," but his lips don't move when he says "something like that."
When Jordan is "tortured" by Venice, she screams out, "Shut up, you little bitch!," but her lips don't move.

Errors in geography

The Exit signs in a New Zealand conference room are USA-style red and white, rather than NZ-style green and white.
The meeting in Geneva is clearly taking place in London. When the car pulls up outside the building, the street signs are those used in the City of London (black Corporation of London on the left side). Also the cars are suddenly driving on the left instead of on the right in the previous scene.

Plot holes

Character error

Jordan says that the US Criminal Code for bribery requires a dollar amount to be a criminal act. This is incorrect. The code states that "anything of value" must be offered as a bribe to a government official to be constituted as a bribe.
When Jordan first meets Naomi she speaks with an Australian accent and not the new York accent that she has for the rest of the film.
Instead of sailing parallel to and in sight of the Italian Riviera en route to Monaco, The Naomi becomes engulfed in a storm in the open sea (in real life, she went down 20 miles off the coast of Sardinia when a 20-knot blow became a 70-knot storm with 35-foot crests). Her captain is not only beyond incompetent, Belfort, despite having no experience, could have piloted The Naomi to Monaco.
At the wedding reception when Aunt Emma arrives, Naomi goes to catch Jordan's attention to introduce them. When it cuts to Jordan dancing, the actor is clearly a body double, cut away and then back and all is correct again.
The guy who introduces Belfort at the seminar (played by the real Belfort) does so while dropping 3 MF bombs. No announcer uses foul language in front of an audience.
