The briefcase, identified as Kerrigan's, that Eliot showed to the Leverage team, is not the same briefcase, also identified as Kerrigan's, that Leary brought to the hospital.
While breaking into the safety deposit boxes, Parker has a bead of sweat on her nose, the appears and disappears.

Factual errors

Eliot said that he found Kerrigan's briefcase "at that intersection", referring to where the accident took place -- however, just before that Hardison showed video of the bad guy disposing of the briefcase in a dumpster several blocks away.
3:31 A fire under the engine will not make the gas tank explode. After the car explodes, there is no fire at the rear of the car where the gas tank would be.

Miscellaneous

When Sophie hits Nate with the cookie sheet, it looks like it's been used to bake cookies, but the underside still has a bright white bar code sticker on it.
At 14:29, a close up of the inside of the safe deposit box shows the bolt of the lock is facing away from the edge, so it could not hold the door shut much less lock it.

Plot holes

It would be ridiculous for Glenn Leary to believe he could get away with shooting a patient with an un-silenced gun in a hospital.
