Doctor Prisoner begins as a gripping, high-concept medical-revenge thriller - fast, stylish, and full of intrigue. The first half of the series hooks you immediately with sharp pacing, morally gray characters, and a fresh blend of hospital and prison politics. Namkoong Min delivers an outstanding performance as Na Yi-je, a surgeon turned prison doctor seeking justice through the very system that betrayed him.
However, as the episodes progress, the narrative starts losing coherence. The show becomes increasingly difficult to follow - especially around Episodes 11-13 - as timelines blur and character loyalties become confusing. At times, it's unclear whether events are unfolding in the present or being revisited from the past. The former medical director's constantly shifting alliances between the Chairman's eldest son and second wife's camp add to the disorientation, reducing tension rather than heightening it.
Several sequences that should feel high-stakes fall flat due to weak realism. The congressman's murder, for instance, shows no visible police investigation, and the Chairman's hospitalization is oddly lifeless - no family, no doctors, just an empty room. The plot too complicated
Innovative plot. The ex lady president is applying to move from jail to hospital now. But as the story develop it become too complicated to understand.
Male lead gives excellent performance, both as a new and subsequent cool doctor. Hope the ending is more viewer friendly.
PS I would like to write a fuller review after viewing the whole drama dubbed in my language. To understand the plot fully makes me raise the score to ten.
An atypical plot on our hero, a doctor in charge of the jail. The story reveals Korea's corrupted jail system while rich prisoners could be free from imprisonment if they are found contracting serious illness.
The acting of all the male lead are amazing. The male lead, Dr Sun and our villian Lee all have impressive performance in the drama. Dr Sun and villian Lee are not new to me. I watch their acting in many drama, KMHM, Mr Sunshine.... They simply create great chemistry with our male lead who also has super acting skills. In comparison, the acting of female lead is not in par with the male leads.
It is still my opinion that it is difficult for non-Korean to grasp the complicated story line. Thanks to the dubbing I can fully appreciate the intricate storyline.

Interesting Perspective !
The series blends three things very smartly. Firstly, the prison environment as a lot of this show happens in/related to prison. Secondly, the medical environment as the leas is the medical doctor in the prison. Thirdly, the mystery aspect as the show is heavily driven by that.
Hence, if you enjoy any part of the three I mentioned above, I have zero doubts that you are going to enjoy this drama.
