After his third strike with the law, smuggling sugar, Sizemore is faced with choosing between a long prison term or becoming a Doctor at Barbee Rehab. What seems at first to be a "Get out of jail free card" will quickly prove to be anything but. Will Doctor Tom be able to clean up the facility and get this cast of Whack-a-doodles in order? Or will he end up in the Nuthouse himself? Is this world real or is it all one big hallucination? Even we don't know.
