Association Among Cardiorespiratory Fitness, Body Fat, and Bone Marker Measurements in Healthy Young Females

Abstract

Context : Serum vitamin D (25-hydroxyvitamin D [25(OH)D]) is important for maintaining bone health, as well as for influencing a multitude of nonosseous effects. Associations between 25(OH)D and other bone marker measurements have not been well studied in relation to obesity and cardiorespiratory fitness in healthy young females. Objective : To determine if there is an association of cardiorespiratory fitness and body fat with bone marker measurements (ie, 25[OH]D and calcium) in healthy young females at peak bone mass. Methods : Females younger than 25 years were recruited as volunteer participants. From July 2007 to October 2007, participants were assessed for cardiorespiratory fitness by measuring maximal oxygen consumption (VO 2 max). In addition, their levels of serum 25(OH)D and serum total calcium were analyzed. Finally, percent body fat was measured using dual-energy x-ray absorptiometry of the whole body. Pearson correlation coefficients were used to evaluate the association of fitness and body fat with bone marker measurements. Results : A total of 59 adolescent girls and young women-were recruited. A statistically significant, positive association was found between VO 2 max and serum 25(OH)D levels (Pearson r =0.36; P <.05), but not between VO 2 max and serum total calcium levels (Pearson r = -0.10, P >.05). Percent body fat was inversely associated with both VO 2 max (Pearson r = -0.70, P <.05) and 25(OH)D levels (Pearson r = -0.44, P <.05). Conclusion : In healthy young females, better cardiorespiratory fitness, as measured by VO 2 max level, and lower percent body fat are associated with higher serum 25(OH)D level. This response is independent of serum total calcium level.

Abstract

Patients with esophageal diverticula may present with a variety of symptoms. Many of these symptoms mimic less serious disease, making diagnosis of esophageal diverticulum difficult for many primary care physicians. Delayed or missed diagnosis of this disorder may lead to more serious problems including gastrointestinal bleeding, aspiration pneumonia, and cancer. Our case series illustrates a variety of symptoms observed among three patients who were diagnosed and surgically treated for esophageal diverticulum. Appropriate diagnosis and management of esophageal diverticulum results in resolution of symptoms and decreased risk of morbidity.

Moyamoya Disease: Case Report and Literature Review

Abstract

Moyamoya disease is a chronic, progressive occlusion of the circle of Willis arteries that leads to the development of characteristic collateral vessels seen on imaging, particularly cerebral angiography. The disease may develop in children and adults, but the clinical features differ. Moyamoya disease occurs predominantly in Japanese individuals but has been found in all races with varying age distributions and clinical manifestations. As a result, moyamoya disease has been underrecognized as a cause of ischemic and hemorrhagic strokes in Western countries. At this time, there is no known cure, and existing treatment options are controversial. The authors describe the case of a 44-year-old African American woman with a history of hypertension, cervical cancer, breast cancer, and stroke who was diagnosed as having moyamoya disease. A review of the literature for the various facets of this condition is also provided.

The Journal of Osteopathic Medicine, founded in 1901 and known for 119 years as the Journal of the American Osteopathic Association, is the premier scholarly, peer-reviewed publication of the osteopathic medical profession. The mission of JOM is to serve as an international forum for the dissemination of scientific literature that incorporates an integrative, comprehensive, patient-centered approach to clinical care and improving health. JOM conducts double-blind peer review of academic research manuscripts from a wide variety of medical specialties, covering the full spectrum of clinical settings in which osteopathic physicians practice. All submissions are vetted by a distinguished group of Section Editors led by Editor-in-Chief Ross Zafonte, DO, and supported by a full Editorial Board.

JOM publishes original research, narrative and systematic review articles (including meta-analyses), clinical practice descriptions, and a very limited number of case reports, all of which are described fully in our Instructions for Authors. JOM is particularly interested in original scientific research that provides quantitative analysis of and contributes significantly to the evidence base underpinning current practices in osteopathic medicine, including but not limited to neuromusculoskeletal medicine. We also accept research submissions documenting trends and changes in education for osteopathic medical students. The ultimate focus of all material published in JOM is to support, enhance, and advance patient care through evidence.
