This expansion adds four to five hours of war-action stealth gameplay and is presented as an in-game virtual reality simulation set in the fictitious city of Anchorage in Alaska. After downloading the content, it appears as a new quest through a radio broadcast. When entering the simulation, the player is stripped off all equipment and weapons, having to resort to supplies in the environment. It deals with the Brotherhood of Steel outcasts who want to infiltrate one of the Vaults to get rare pre-war technology. But to pass the security system, the infiltrator should break through the virtual training game, set in Anchorage and representing the defense against the Chinese Communist forces. The player assumes the role of the infiltrator.

The second downloadable content for Fallout 3, The Pitt takes players to the ruins of Pittsburgh, Pennsylvania - now a smog-filled industrial hellhole brutally ruled by a gang of raiders. The streets of "The Pitt" are also home to captured slaves from across the Wasteland; forced to work in large smelting factories, where they make the very weapons and ammo the raiders use to maintain control.

Point Lookout is the fourth downloadable expansion for Fallout 3, and takes the player to the misty swampland along the coast of Maryland. The town of Point Lookout is a unique area in the Fallout universe, in that it has not been directly hit by bombs or affected the subsequent warfare. Instead, it has suffered decades of neglect and isolation.

The fifth and final downloadable expansion for Fallout 3, Mothership Zeta begins with the player picking up a strange radio transmission from the Wastes. The signal leads to a crashed alien scout ship that can be discovered in the regular game - only this time, you won't be allowed to leave. An alien beam pulls the player into the orbiting mothership, where they must band together with abductees from different periods of Earth history to escape.
