- Four years after the death of Isabel Aretas, MPD Detective Mike Lowrey marries his physical therapist, Christine. During the wedding reception, his partner, Detective Marcus Burnett, suffers a mild heart attack and falls into a coma. While unconscious, Marcus has a vision of their late Captain Conrad Howard, who reassures him that his time has not yet come.
When Marcus wakes up, he becomes convinced he cannot die. Shortly afterward, a news report accuses Captain Howard of being tied to drug cartel corruption, tarnishing his legacy. Determined to clear Howard's name, Mike and Marcus enlist the help of Captain Rita Secada and her boyfriend, mayoral candidate Adam Lockwood. The investigation leads them to Mike's estranged son, Armando Aretas, who is incarcerated for Howard's murder. Armando reveals that Howard was not corrupt but was close to exposing those who were. To protect themselves, the real culprits hired Armando and Isabel to kill him. Armando offers to help identify the conspirators.
Meanwhile, a fail-safe video left by Howard is triggered when the conspirators attempt to access his computer. The video, sent to Mike and Marcus, warns of widespread corruption in law enforcement and includes the cryptic phrase "Coke bottle." Following the lead, they track down their former hacker colleague, Fletcher, who provides them with a QR code hidden in wall art that unlocks a second video from Howard. Before they can act on it, assassins hired by the conspirators ambush them at Fletcher's club, killing Fletcher.
In prison, Armando is attacked by disguised inmates but manages to fend them off. Mike and Marcus arrange for his transfer to a more secure location in Miami. However, the conspirators infiltrate the transfer, killing the pilot and guards and causing the helicopter to crash. Though Mike, Marcus, and Armando survive, they are now fugitives, hunted by law enforcement and bounty-seeking criminals. U.S. Marshal Judy Howard, Conrad's daughter, vows revenge on Armando for her father's murder.
With assistance from AMMO members Dorn and Kelly, Mike and Marcus decipher Howard's clues, uncovering the identity of the mastermind behind the conspiracy: James McGrath, a former Army Ranger turned DEA agent. McGrath was coerced into working with the cartel after being captured and tortured in South America. McGrath retaliates by kidnapping Christine and Callie, Judy's daughter, and sending assassins after Marcus' family. However, Marcus' Marine son-in-law, Reggie, successfully defends them. Meanwhile, Rita and the team apprehend Lockwood, who is revealed to be working with McGrath.
Using Lockwood as bait, the team lures McGrath to an abandoned alligator-themed amusement park, leading to a fierce gunfight. During the chaos, Lockwood betrays the team but accidentally falls into the water, where he is devoured by an alligator. Mike confronts McGrath, who takes Christine and Marcus hostage, forcing Mike to choose between them. Trusting Marcus' newfound belief that he "cannot die," Mike shoots Marcus in his bulletproof vest, creating an opportunity to rescue both hostages and kill McGrath.
Amidst the turmoil, Judy corners Armando but hesitates when Callie reveals that Armando saved her life. Reluctantly, Judy spares him, allowing him to escape.
With the corruption exposed and their names cleared, Mike, Marcus, and Judy reunite with their families. They celebrate with a picnic in the park, where Mike and Marcus grill food alongside Reggie, cherishing their newfound peace.
