While the doctors are in a video chat with the pregnant woman and driver, they have the tablet in landscape mode, while the phone in the car is in portrait more. The images seem correct on both screens, but no video chat app that is currently available (or ever been), has been able to display a video feed like that. The moment one side of the chat holds the phone/tablet in another mode than the other side, both sides will see the other video sideways to keep the camera ratio correct, or in some cases, if can be corrected, having black side bars.
