In the party sequence early in the film, we first see (in long shot) a woman who is seated on a sofa with George Hansen behind her. The woman is wearing a blue dress, the top of which is open in front and her right breast is clearly and fully exposed. In the next shot (from a different angle) we see that her breasts are tucked neatly into the dress.
When Mike and George go to relieve the two men at the safe house, we see the four cameras on top of the building all fixed on the same plate that spins back and forth. But when we see the monitors inside the building the video shows the scenery panning in opposite directions from each other instead of being in sync like the cameras are.
When Burt Young's taxi hits another car, the hood flies totally open making it impossible to drive. With the next shot it has been moved down again.
In a collision during the taxi chase scene, the front end of the taxi is severely crumpled with the hood popped off and twisted, but in some ensuing shots it is either undamaged or only slightly crumpled.
When the boy throws Mike's cane to him he throws it pointed end in front. When it cuts to Mike catching the cane it is the rounded end in front.
Mike Locken has plaster full-limb casts cut off his leg and arm. These plaster casts are semi-permanent casts meant to immobilize broken bones for long periods of time, from many weeks to several months. However, Mike's casts also cover surgical wounds from the shattered bones being surgically repaired. These surgical wounds have dressings that need changed regularly, probably daily, and would never be so permanently covered.
Jerome kills the police officer (who he later reveals is not one by his choice of firearm) and the group abruptly leaves the area without concealing the body.
For professionals this doesn't make much sense.
Even if the "officer" was not one, it could take as long as several days for the police department to identify him as being a fraud. In the interim, the police would proceed under the assumption that the individual was a police officer and they would hunt the shooter with a great deal of alacrity. Hiding or concealing would have been prudent.

In the cab station, Jerome said the chrome plated pistol. Pistols are nickel plated not chrome plated. First the pistol is copper plated and then the nickel is applied over the copper.
The opening disclaimer mentions an interview with character Lawrence Weyburn that took place on September 31, 1975. There are only 30 days in September, so this is likely an indication that this is not a disclaimer at all.
Mac says out of the blue during the taxi chase scene that something is the matter and he stops the taxi and gets out and looks under the car and finds the bomb stuck under the car. However, there is absolutely nothing that could have clued him in to the bomb's existence. It was stuck to the frame and couldn't have affected the performance of the car as he implied.
When Mike's semi-permanent plaster casts are finally removed after many weeks or even several months, they cover surgical wound dressings with stitches under them, which are both also removed. Not only do the dressings show no signs of being in place for so long (they look very fresh and clean), but also the skin under the stitches shows no sign of the surgical incision at all, the skin is original and unbroken.
One of the characters is shown before, during, and after major orthopedic surgery on his elbow and knee. Following surgery, both the entire arm and entire leg are encased in close-fitting gypsum casts. The intact casts are taken off many days later. Even with today's medicine, this would be impossible: injury and surgery to those joints would cause a great amount of unavoidable swelling that would make close-fitting casts detrimental and torturous. If casts are used today, ordinarily the area around the joint is cut out to accommodate swelling.
In the freighter graveyard scene, Miller holds a submachine gun identified as an Uzi. In fact, it is an Ingram Mac-10 with a cosmetic attachment to make it appear superficially like an Uzi. Furthermore, when first shown in Miller's hands, the attachment is on upside-down, putting the large front sight assembly on the bottom of the weapon instead of the top. In the next shot, the attachment is correctly oriented.
During surgery, a crew member and light source are reflected in the doctor's glasses.
In the beginning as they are driving to the safe house the are driving westbound on the Richmond-San Rafael Bay Bridge. Then they jump to shots of them driving eastbound, and then finally to them exiting the north side of the Golden Gate Bridge.
After escaping the car chase, they are seen driving along the shoreline of San Francisco Bay. This road is actually coming from Fort Point, and there is no way to get to this road from the direction they are coming from.
As they are sailing to the Suisun Bay Mothball Fleet, they are sailing under the western span of the Bay Bridge, then you can see the Golden Gate Bridge off in the distance, which would have been shot from Berkeley, and then they are sailing along the San Francisco water front.
At the end of the movie, after they have left the Suisun Bay Mothball Fleet, they are actually on the west side of the Benicia-Martinez Bridge sailing eastbound. You can see the Southern Pacific Railroad bridge in the back and you can make out the Mothball fleet off in the distance.
During the several minute police chase, the cab containing the protagonist ends up traveling from Chinatown to Hunters Point/Bayview ( the old naval base is shown) in a short period of time. While the distance between the two areas is only 7 miles, to travel requires driving across the most dense parts of San Francisco and could normally take as long 45 minutes.
Mac, the driver, stops the vehicle because he "feels something is wrong" and discover a bomb. He is underneath the cab trying to remove it when the highway patrolman stops to investigate why the cab is parked alongside the road.
The timer is shown to be electronic and the bomb was well-anchored to cab. SIince it wasn't making any noise, how did Mac discover it?

Jerome, a weapons expert and salesman, is shown incorrectly inserting a magazine into an Uzi sub machinegun when the group is sailing into Suisun Bay to hide out. Worse, prior to trying to insert, he looks at the magazine, then tries to insert having to spin it around twice before he can do so.
Weyburn tells Lochen to sail to the mothball fleet located in (phonetically) "SOO-SEE-OON" Bay. It is actually pronounced "SOO-SOON" Bay.
