Edwige Fenech's real voice is used throughout the entire film in the English-language version (shot in 100% sync sound). This is a rarity for her speaking in her own voice as her previous roles are dubbed by professional voice actresses.
Although uncredited, composer Pino Donaggio's song "Wild Dawn Nights" from his earlier film SAVAGE DAWN is reused here in the background during the party scene after the opening credits sequence.
The scene of Inspector Datti (Donald Pleasence) running and screaming in the town square was filmed guerilla style with hidden cameras all around the square. It was shot in one take.
Director Ruggero Deodato said in an interview that "Off Balance" (aka "Phantom of Death") is his least favorite film he ever directed as he felt no connection to the plot. He admits he only took the job as he needed work due to the Italian film industry was beginning to dry up in the late 1980s.
Ruggero Deodato admits that actress Edwige Fenech was forced upon him to use as she had a good working relationship with production manager Fabrizio De Martino. He would not have chose her for the part as he felt she was too old at the time. He even decided to film a love scene between her and Michael York while they were covered in sheets as he felt she was too out-of-shape. This did not please the producers as they had promised distributors nudity but they didn't have enough time or budget to re-shoot the scene.
