When the car with Alice is pushed over the edge, Alice wakes up when trees whip the sides of the car and she gets out. Later when Bruce looks over the edge at the car crashed and on fire, there are no trees on the hillside.

Swapping the x-rays in the dental records would not be enough, the dentist would have to rewrite the entire patient notes to match the dental work in the x-rays.
