About us

Signify Health, part of CVS Health®, exists for one simple purpose: to build trusted relationships that make people healthier. We do this by bringing comprehensive and personalized health evaluations and preventive screenings right to health plan members’ front doors. Each year, our experienced clinicians, supported by innovative technology, visit millions of people nationwide with the goal of improving health outcomes, closing gaps in care and providing connections to the right care for their unique needs. From just one easy appointment, health plan members receive a detailed picture of their health, with the time to get their questions answered. After the visit, Signify Health shares a comprehensive report with the member, their primary care provider and their health plan. Our proactive approach enables members, health plans and health care providers to act on the information we provide efficiently and effectively. Ensuring a more connected, effective care experience for all.

  • Early screening is crucial for protecting memory and cognitive ability! With 92% of older Americans living with undiagnosed mild cognitive impairment (MCI), and more than half of older adults who suspect cognitive decline not talking to a health care professional, it's time to take action. Early detection can help slow disease progression and support older adults in maintaining their independence.
