Romero Blvd. Manila Harbour Centre, Radial Road 10 Vitas, Tondo, Manila Dear Sir: I, __________________________, would like to inform you that permission is hereby granted to my son/daughter ___________________________________ to participate in the Industry Internship Program which is an academic requirement in the Bachelor of Science in Chemical Engineering, degree of Pamantasan ng Lungsod ng Maynila (PLM). I understand that my son/daughter will undergo the said program this Summer, School Year 2012-2013 from _______________ until the completion of the required 240 training hours, which will be conducted in Philippine Foremost Milling Corporation located in Vitas, Tondo, Manila. I am aware that the Professor-in-Charge will monitor the performance of the student, and that the immediate superior within the company will take all the necessary precautions to keep the student safe.
