WAIVER FOR PRACTICUM

This waiver is being executed in connection with the Practicum that Allen Grace A. Salazar a
B.S. Chemical Engineering student from Adamson University has requested and which
DIC Philippines, Inc. has accepted and confirmed.

The student/practitioner has been granted permission by Ms. Ma. Theresa Timoteo and she therefore
agrees/conforms to the conditions, rules and regulations of the institution and she accepts and agrees to
comply with the company’s rules and regulations. It is also understood that there is no employer-
employee relationship between the student practitioner and the company.

That while performing the practicum requirements, the company is free and harmless from any liability
whatsoever during the period of practicum or immediately thereafter while on the way to and/or from the
office of the practicum center, to perform the practicum or while performing the same or while in the
premises thereof.

This waiver will be in effect for the duration of the Office Practitioner schedule where at least 240 hours
will be spent in the company.
