Lot _____ Mansanas St., Bilog Balangkas Valenzuela City after having been duly sworn
to in accordance with law, depose and state THAT;

1. A demand letter was sent to Mr/Ms. ______________________________ of Blk


____ Lot _____ Mansanas St., Bilog Balangkas Valenzuela City demanding
payment of his/her arrears in the monthly amortization under the Community
Mortgage Program (CMP). Said demand letter is hereby attached as Annex “A”.

2. However, there is no proof of receipt of the said demand letter by Mr/Ms.


______________________________ Attached herewith is the certification issued
by the post office involved starting, among others, that Mr/Ms.
______________________________ did not receive the demand letter sent by
Social Housing Finance Corporation (SHFC) as Annex.

3. To ensure that Mr/Ms. ______________________________ is notified of his


arrears in the payment of his monthly amortization, I caused the posting of the
demand addressed to Mr/Ms. ______________________________ in three (3)
conspictions public places where the association is located as required by SHFC
Corporate Circular no. 16-0-16, Series of 2016 regarding the Amendments to the
Guidelines on Substitution of MBs;

4. I am executing this Affidavit to attest to the truth of the foregoing facts; and

5. Further, I am executing this affidavit in compliance with the above-mentioned SHFC


Corporate Circular.
