AFFIDAVIT OF CORRECTION

I, MELINDA V. FABIAN, of legal age, married, Filipino, and a resident of


Tetuan, Zamboanga City, Philippines, after having sworn in accordance with law, do
hereby deposes and states:

1. That my official name as appearing in my Birth Certificate is Melinda


G. Villanueva;

2. That, however, I used the name MELINDA NURMINA VILLANUEVA


when I went to the Middle East as an overseas contract worker;

3. That when I got married to Erico Basilio A. Fabian on November 12,


2005 at Zamboanga City, I used the name MELINDA G. VILLANUEVA;

4. That the name “MELINDA NURMINA V. FABIAN” and “MELINDA V.


FABIAN” is one and the same person;

5. That I am executing this affidavit of discrepancy to attest to the


truth of the foregoing facts.

AFFIDAVIT OF CORRECTION

I, EDITHA Y. CALIBAT, of legal age, married, Filipino, and a resident of


Navarro Court, Sta. Maria, Zamboanga City, Philippines, after having sworn in
accordance with law, do hereby deposes and states:

6. That I am one (1) of the children of the late Gaudencio L. Yenko who
died on February 2, 2014 at the age of 87;

7. That upon his death, I caused the processing of his Death Certificate;

8. That in the course of said processing, I could not exactly recall the
name of his parents, hence, I gave the name “PONCIANO YENKO and
PACITA LAZARO YENKO” as his father and mother respectively;

9. That lately, I came across the Marriage Contract of my father where I


eventually discovered that his parents actual name are as follows:

Father – Francisco Yenko


Mother – Presentacion Lazaro

10. That my poor memory of their names are due to the fact that they died a
long time ago and that I have not met them personally;

11. That I am executing this affidavit of discrepancy to attest to the truth of


the foregoing facts and to explain the correction of entries of my grand
parents name.
