Incident Report

Caesar Rey C. Estopito, a former employee of Philippine Vending Corporation, is submitting an incident report to explain expired juices found at a Nestle factory. According to the report, expired juices from outside machine accounts were brought to the factory by a route man in January or February and stored in Mr. Estopito's cabinets since they could no longer be used elsewhere. Mr. Estopito did not request these additional juices and they took up two full cabinets. He is writing to clarify that the expired juices were not from his regular withdrawals and to request his final pay, 13th month pay, and unused leave pay since resigning from the company.

SUBJECT : Explaination regarding the incident report

Please state all important detail such as employee concerned, date and time, place or other matters
necessary to fully explain the incident.

I am Caesar Rey C. Estopito, a resigned employee of Philippine Vending Corporation that wanted to explain
regarding the issue of expired juices on Nestle Factory upon my exit. I was the assigned static when that
happens but that juices doesnt come from my withdraws. It was originally from the MRNK machine accounts
outside the factory that stops dispensing pineapple and lemonade juices and it happens that the routeman
brought it to the factory and ask me to put it there since they cant use that anymore and it says that "it
cannot be returned once it is already withdrawed". At first I disagree of what the routeman is asking but
since he told me that there is no space for that juices on the truck since he has a lot of withdraws as well, I
was convinced and agreed to put that on my cabinets inside the factory. The juices that was brought there
wasnt already in their original boxes. It was already putted in a box of milo and with random of flavors
considering that is a pullout products from other machines adn maybe some of them are already expired. For
me, it is very unfair if ill be the only one to took the blame for that since on the first place I wasnt requesting
for that products, the routeman just wanted to put it there because he can no longer use it for the machines
outside. The juices were brought to the factory around January or February of this year so if there are jucies
that is already expired for the year of 2018, then it is not from my withdawn juices. I commonly ask for 1 box
of powder juice for 2 to 3weeks with 4 varities of juices so that comes up with atleast 4 boxes of juices every
3weeks or so but when that additional juices brought from outside and putted into the cabinet of my
designated machines, it consumes 2 cabinets that are full of juices from outside. I know I have lapses when it
comes to reporting it to the immediate head since I thought the routeman already took the responsibility of
reporting it since it was his withdraws and not mine.

In line with this, I am afraid I couldn't get my last pay, the half of my 13th month and also my unused leaves
from almost 2years in service of this company. It is the reason why I am writing this explaination to clear my
record and clearance.
