INFORMATION

The undersigned Assistant City Prosecutor accuses MARIO ________, a resident of 123
Atate, Palayan City, Nueva Ecija, Philippines of the crime of RAPE, defined and penalized
under Article 266-A, par 2, of the Revised Penal Code, as amended, committed as follows:

That on or about the 1st day of March 2018, in the City of Palayan, Nueva Ecija, and
within the jurisdiction of this Honorable Court, the above named accused, being the cousin
of the minor victim, through force and intimidation, did then and there, willfully,
unlawfully, and feloniously commit an act of sexual assault upon the person of MARISSA, a
minor 12 years of age, by then and there inserting his middle finger into the victim’s vagina
against her will.

That the victim suffered actual and moral damages in the amount as may be
rewarded by the Honorable Court under the provisions of the New Civil Code of the
Philippines.
