Malinaw Dr., Makati City , after having been duly sworn to in accordance with law, do hereby
depose and say that: PERSON’S
BONA FIDES
1. I am the Supervisor of Marketing Department of AHA Fruits Company ;

2. On January 7, 2019 , I was tasked to serve a copy of the Summons addressed to


General Food Products Distribution Co through its office address at 6th Floor Magellan
Building, Ayala Avenue, Makati;

3. In compliance with the said order, I served the same at its office in 6th Floor
Magellan Building, Ayala Avenue, Makati on such date at 10:30 A.M.;

4. Upon service of the said letter to Atty. Kap- Uy named as one of its resident
agent, the latter after reading and accepting the same, audaciously refused to sign the
receiving copy thereof;

5. Because of such refusal, I noted such development on our receiving copy and
sign the same after affixing the date and time of such service;

6. I am executing this document to attest to the truth of the foregoing;
