CHECKLIST FOR SUPPOSITORY

1 PURPOSE
 To stimulate defecation
 To treat constipation.
 To relieve gaseous distension.
 To introduce peristalsis
 To reduce temperature
2 PREPARATION OF ARTICLES –
 Suppository
 Mackintosh and towel
 Kidney tray and paper bag
 Bed pan
 Screen
 Gloves
3 PRELIMINARY ASSESMENT
 Check the diagnosis
 Check the date and time of surgery
 Check the nature of enema ordered
 Check for any lesions in rectal or perineal area
4 PROCEDURE
PREPROCEDURAL STEPS:
 Explain the procedure to the child according to his
level of understanding if old enough if not, then to
parents
 Put on the curtains/ screen.
 Place mackintosh and towel under patient’s buttock
 Keep bed pan under the bed
 Position the patient in comfortable position usually
in left lateral position.
 Perform hand hygiene. Wear gloves
INTRAPROCEDURAL STEPS:
 Remove the suppository from its package and hold it
in right hand between two fingers.
 Separate the buttocks with the left hand and insert
the suppository into the anus.
 Once it has passed the external sphincter, advance it
beyond the internal sphincter, pushing it with index
finger and in infants with little finger.
 Patient should be instructed to retain the
suppository as long as it is comfortable for about 15
-20 minutes.
 Provide the patient with bedpan. If the child is old
enough to go to bathroom, assist him/her onto
bathroom.
 Remove and discard the gloves. And perform hand
hygiene.
5 AFTER CARE:
 Take all the articles to the utility room and discard
the waste, clean the articles and store it in proper
place.
 Wash hands.
 Return to patient and evaluate patient’s condition.
 Send the specimen if any to the laboratory.
6 RECORDING AND REPORTING:
 Record the type of suppository given, time of
administration and characteristics of results.
 Report if the patient fails to defecate to the
physician.
