PERINEAL CARE/FLUSHING

 Prior to the procedure:
      Check and verify the Identity of

          Ability of the patient if pt. can
          move independently, Irritations,
          Excorations, Swelling at the
          perineal area, Excessive discharge,
          Odor, Pain and Incontinence)
 Gather and prepare materials needed:
    Bath Basin
    Soap
    Two or three Washcloth
    Cotton (soaked in water and soap)
    Dry bath towel
    Bath blanket
    Water proof pads
    Toilet tissue
    Lotion or ointment
    Disposable gloves
 Wash Hands and Consider the Use of PPE
 (In accordance to Health Care Setting
  Protocols)
 Provide Privacy by drawing the curtains
 and closing the door
      Assist the client to the Supine
          position
          Female: Knees flexed and spread
          well apart;
          Male: Knees slightly flexed and
          hips slightly externally rotated
 Fold the top linen to the foot of the bed
 and fold the gown up to expose the
 genital area. Place the bath towel under
 the client’s hips
 Determine whether the client can wash
 the perineal area independently. If cannot
 do so, drape, wash and dry the upper
 inner thighs. Inspect the perineal area.
 Wash the Perineal-Genital area:
        Female: Clean the Labia Majora,
        then spread the folds of the Labia
        Majora and Labia Minora. Use
        separate    quarters     of    the
        washcloth for each stroke and
        wipe from the pubis to the rectum
         Male: Wash and dry the penis and
         the scrotum. The posterior folds
         of the scrotum may need to be
         cleaned when the buttocks are
           cleaned.
    Rinse the area well. Place the client
     on a bed pan and use solution bottle
     to pour warm water over the area
    Dry the perineum area thoroughly
    Inspect the perineal orifices
 Assist the client too turn unto side facing
 away from you. Clean the anus with toilet
 tissue before washing it, if necessary. Dry
 the area well.
    Remake the bed or change the linens,
      if needed, remove the bath blanket
    Reposition the patient comfortably
 Remove and discard gloves. Perform hand
 hygiene
 Assist the client with grooming aids and
 putting on a clean gown
 Remove, clean/ disinfect and return used
 equipment and materials. Place soiled
 linens in the laundry bag. Dispose trash in
 proper bins.
    Perform hand hygiene
    Document the procedure and any
      unusual findings.
