AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, RODOLFO E. PONO and MARRIELE B. DOÑA both Filipinos, of


legal ages, and residents of Poblacion, Catmon, Cebu, after having been
duly sworn in accordance with law, hereby depose and state:

1. That we personally know the person of “LEIZEL URSAL OCHEA” who


was born on 9 March 1969 for being our neighbor and close acquaintance;

2. That we know for a fact that the person by the name of “LEIZEL
URSAL OCHEA” as appearing in her Certificate of Live Birth from the
Philippine Statistics Authority (PSA) and the name “LIEZEL URSAL OCHEA”
as appearing in her Marriage Certificate, refer to one and the same
person and that her true and correct name is LEIZEL URSAL OCHEA;

3. That to remove confusion as to the two names, we hereby declare


that the name “LIEZEL URSAL OCHEA” and “LEIZEL URSAL OCHEA” refer to one
and the same person;

4. That we are not in any way related to any of the foregoing


person within the fourth civil degree either by consanguinity or
affinity, nor do we have any pecuniary interest in this matter
whatsoever;

5. That we are executing this affidavit in order to attest to the


truth of the facts aforementioned, and for any legal purpose this may
serve.
