AFFIDAVIT OF TWO DISINTERESTED PERSONS

We, Clarita B. Sudaria and Corazon S. Sitoy both Filipinos, of legal ages, and residents
of San Jose, Catmon, Cebu, after having been duly sworn in accordance with law, hereby depose
and state:

1. That we personally know the person of “WENIFRED ORLANES BOTER” who was
born on 21 September 1956 at Catadman, Catmon, Cebu for being our neighbour and close
acquaintance;

2. That we know for a fact that the person by the name of “WENIFRED ORLANES
BOTER” as appearing in his Certificate of Live Birth both Local and from the Philippine Statistics
Authority (PSA) and the name “WENIFREDO ORLANES BOTER” as appearing in his driver’s
license with no. A1693487 duly issued by the State of California, United States of America, his
American Passport with no. 528605868 and his other pertinent documents, refer to one and the
same person and that his true and correct name is WENIFRED ORLANES BOTER;

3. That to remove confusion as to the two names, we hereby declare that the name
“WENIFRED ORLANES BOTER” and “WENIFRED ORLANES BOTER” refer to one and the
same person;

4. That we are not in any way related to any of the foregoing person within the fourth
civil degree either by consanguinity or affinity, nor do we have any pecuniary interest in this
matter whatsoever;

5. That we are executing this affidavit in order to attest to the truth of the facts
aforementioned, and for any legal purpose this may serve.

IN WITNESS WHEREOF, we have hereunto set our hands this ___ July 2019 at Cebu
City, Philippines.
