AFFIDAVIT OF CONSENT
FOR A MINOR TO TRAVEL ABROAD
I, , of legal age, Filipino Citizen, and with residence and postal address at
No. 56, PNB Ville, Bakakeng Central, Baguio City, Philippines, after having been
duly sworn in accordance with law, do hereby depose and state:

1. That I am the father of , who is THIRTEEN (13) years old as she was born on
, , likewise a citizen of the Republic of the Philippines;

2. My daughter is schedule for a foreign country trip to HONG KONG on the 1st
week of June, 2019 and to be accompanied by her maternal grandparent,
ROSARIO; A copy of her Philippine Passport bearing the No. EC3176806 is
hereto attached and marked as Annex “A” for reference.

3. That for this purpose I am giving my full consent to my daughter for her to
travel abroad;

4. That for this purpose, me and her mother have sufficient and adequate
financial capacity to support and defray the said travel and do hereby
undertake to finance their trip abroad from the application fees, airline
tickets, board and lodging, pocket money and such other expenses that they
may incur so that they would neither be a burden to the state nor at their
place of destination at any time during their trip abroad;

5. That there is no criminal case pending in court against them nor were they
been charged of subversion, rebellion, insurrection or any crime or offense
involving moral turpitude in any court of the Philippines;

6. That I am hereby giving my full consent to her maternal grandparent, Ms. to


accompany or escort my daughter, , in her travel abroad; and

7. That I am executing this affidavit to declare the truth of the foregoing facts
and for whatever legal purpose it may serve.
