AFFIDAVIT OF DISCREPANCY and EXPLANATION

I, ____________________, Filipino citizen, of legal age, married with residence address


at _________________________, after having been duly sworn in accordance with law,
hereby depose and say:

1. That I applied for the issuance of a Philippine Passport sometime in the year
2000 at the Department of Foreign Affairs in the City of San Fernando, La
Union;
2. That I have submitted all the necessary documents for the issuance of the
same, including documents stating that my place of birth is in
_______________;
3. That in 2010, I procured from the then National Statistics Office a Copy of my
Certificate of Live Birth for the purpose of applying for a marriage license,
and I was surprised to learn that my place of birth was in
__________________;
4. That such discrepancy was an honest mistake because I have always led to
believe that I was born in the _________________ and that
________________was my parents’ hometown;
5. That I am now declaring my intent to use __________________________ as my
place of birth in my application for renewal of my Philippine Passport and all
other pertinent document;
6. That the previous declaration of __________________________- as my birth
place was not done bad faith nor for other deceitful purposes;
7. That I am executing this Affidavit to clear any discrepancy which may arise
from personal and official documents.

FURTHER AFFIANT SAYETH NONE.
