AFFIDAVIT OF LOSS OF BIR BOOKLET-RECEIPT

I, ______________________. , of legal age, married, Filipino, and a resident of


Barangay, Balabag, Boracay Island, Malay, Aklan, after having been duly sworn to in
accordance with law, do hereby depose and say:

1. That I am the sole proprietor of ______________________ (establishment),


located at Bolabog Road, Balabag, Boracay Island, Malay, Aklan;

2. That the said establishment is duly registered with the Bureau of Internal Revenue
(BIR), with _________________;

3. That subsequently, the establishment have printed booklets of receipts, one booklet
of which is Booklet No. 15 with O.R. No. 0701 to 0750;

4. That sometime around the first quarter of 2019, I discovered that our said Booklet
No. 15 with O.R. No. 0701 to 0750 were missing;

5. That despite diligent efforts exerted to locate Booklet No. 15 with O.R. No. 0701
to 0750, I could not find the same such that I now consider it lost beyond recovery;

6. That I am executing this affidavit to attest to the truth of the foregoing facts for all
legal intents and purposes and in particular.
