AFFIDAVIT OF PARENTAL ADVICE TO MARRIAGE

I, LOUELLA C. SIASON, of legal age, married, Filipino, and a resident of Lot 38,
Block 13, Woodstock Subdivision, Purok Matahum, Brgy. Tangub, Bacolod City after having
been duly sworn to in accordance with law, hereby depose and say that:

1. That I am the aunt of AYANA JOY I. CABALHIN, 24 years old and a


resident of Block 13, Lot 28, Woodstock Subdivision, Purok Matahum, Brgy.
Tangub, Bacolod City;

2. That since the death of her parents as evidenced by the death


certificates attached herewith, I have been acting as her sole guardian up to
present;

3. That my niece AYANA JOY I. CABALHIN is intending to enter into


marriage with a MALAYSIAN CITIZEN by the name of Mohd Fariq Fadhli Bin
Abdul Malek, presently residing at 126, S2 H5, Park Avenue, Seremban 2,70300,
Seremban, Negeri Sembilan,Malaysia;

4. That at present, she is applying for a Marriage License before the


Philippine Embassy of Malaysia at Kuala Lumpur, Malaysia, and is being required
to submit a parental consent/advice pursuant to the requirement of the Family
Code of the Philippines;

5. That only me can give such parental consent/advice following the


death of her parents;

6. That such parental advice has been given to her favorably for the
reason that she is still single and has all the qualifications to marry without legal
impediments;

7. That this affidavit is executed to attest to the truth of the


foregoing facts and for whatever purpose this may serve.
