AFFIDAVIT OF GUARDIANSHIP
I, ARNOLD C YBANEZ, married, of legal age, Filipino, and a resident of
Talomo, Davao City after being duly sworn to in accordance with law, do hereby
depose and state, that:

1. I am the uncle of ANGELYN P YBANEZ;

2. I am related to her because her father is my brother;

3. Her father, ANTONIO YBANEZ died in 2014 and ANGELYN


has no contact of her mother since 2014;

4. I have been the standing guardian of ANGELYN P YBANEZ


since 2014 until present;

5. I am executing this Affidavit in good faith, for the purpose f


giving her consent to marry;

IN WITNESS WHEREOF, I have hereunto set my hand this July 26, 2016,
