That I, _______________, of legal age, Filipino, widow and a resident of


__________________, Cebu City, Philippines, after having been duly sworn to in
accordance with the law do hereby depose and say:

1. That I am the biological mother of ____________________, my son with my


late husband ______________________;

2. That when my husband died on March 9, 2018 at


________________________, my son was still a minor which by operation of law made
me the natural and legal guardian over his person and properties;

3. That in my capacity as the surviving wife of the late


_________________________ and as the natural guardian of my son
_______________________, I executed a Deed of Extrajudicial Declaration of Heirs and
Settlement of Estate of Deceased Person known as doc. no.; page no.; book no. ; series of

years old;

4. That I am executing this affidavit in order to prove that I am the guardian of my


son during the time that he did not yet reach the age of majority and I executed a
document of settlement referred under par. 3 hereof for the estate of his father, in my
capacity as his legal and natural guardian;

5. That this affidavit is executed with the full awareness of the consequences of
falsely declaring material fact or facts therein, in support of our issuance of title and tax
declaration under our names over the condominium unit and parking.
