To whom it may concern,

Greetings of peace.

We are the students of the Polytechnic University of the Philippines San Pedro
campus. We are currently conducting a business analysis research entitled
Comparative Analysis on the Effects of Costumer Service to Costumer Retention
Between Jollibee Festival Mall Alabang and Jollibee Pavilion Mall as partial
fulfilment of our subject course Business Analytics. This subject course is under the
supervision of our professor Ms. Jemimah Velasco.

In the line that, we would like to ask for your permission to allow us to conduct a
survey among your costumers and series of interview with your selected store
personnel.

The survey will last approximately 10-15 minutes. Participation in the said survey
is entirely voluntary and there are no known anticipated risk in the said survey. Whereas
the interview could be arranged at time most convinient to your store personnel
considering there would be no conflict with our class schedule.

All information wil be treated with upmost confidentiality and will be used solely
for academic purpose.

If you wish to have a copy of the interpreted data, we will br glad to send you an
exclusive summary. Moreover, of you would be interested to go into the details, an
electronic (ex. PDF) or a hard copy of the entire research can be readily made available
for you.

If consent is granted, kindly sign below acknowledgeing your consent and


permission for us to do the survey and interview. We can also give a copy of this letter if
the situation requires.
