AFFIDAVIT OF LOSS

I, ROSITA DC DELA ROSA, of legal age, Filipino and resident of


Longos, City of Malolos, Bulacan, after having been duly sworn to in
accordance with law hereby depose and state that:

1. I am a holder of PWD ID with number 031410025-134 issued by


the City Government of Malolos, Bulacan;

2. Said PWD ID was lost/misplaced. Diligent efforts exerted to


locate for the same but proved to be futile and beyond recovery;

3. I am executing this affidavit to attest to the truth of the foregoing,


and for whatever legal purposes the same may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this 15 th day


of October, 2019 here in Malolos City, Bulacan.
