AFFIDAVIT OF LOSS
I, JONJON CAPILI CALALO, of legal age, married, Filipino, and
a resident of 61 Rosal St. Miguelito Homes, Valenzuela City, after having
been duly sworn to in accordance with law do hereby depose and state,
that:

1. I am a holder of a company Identification Card issued by the


Direct Lines Sales Unltd. Inc.;

2. On November 6, 2019, at 12:30 P.M., while riding my motorcycle


on my way back to our office in Malinta, Valenzuela City, I
noticed that the subject Identification Card was missing;

3. I suddenly stopped and alighted from my motorcycle to check and


look for my Identification Card but could no longer find the same;

4. The said Identification Card might have been unnoticeably


dropped somewhere in Obando, Bulacan while I was riding my
motorcycle and nowhere to be found;

5. I have exerted diligent efforts to locate the said company


Identification Card but all my efforts proved to be all in vain; and

6. I am executing this affidavit to attest to the truth of the foregoing


statements and for all legal intent and purposes it may serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 6 th
