CHILD SUPPORT UNDERTAKING

KNOW ALL MEN BY THESE PRESENTS:


That I, BRENDON MALAPO, of legal age, Filipino, married and with office and postal
address at DSWD-Sikap Foundation, Inc., Santa Cruz, Laguna in my capacity as natural
biological father of the four month old minor child ADRIAN EMMANUEL ESCANO hereby
voluntarily undertake to give support to my said child. The details are given below:
1. The mother of ADRIAN EMMANUEL ESCANO is JOURNNIE MARRIANE ESCANO who
was still a minor at the time of conception and birth of Adrian Emmanuel Escano.
2. I agree to provide financial support of PHP10,000.00 per month to cover the needed
funds for the rearing and promotion of the welfare of ADRIAN EMMANUEL ESCANO.
This amount will be directly deposited and credited to designated bank account of
Maria Fe E. Escano, the mother and legal guardian of Journnie Marianne Escano.
3. In addition to the said amount, I also agree to shoulder the expenses for baptismal,
medical and immunization requirements. When the child reaches six (6) months, the
expenses of the child will accelerate by twenty percent; and when the child reaches
school age (4 years), the expenses of the child will be doubled, as we have likewise
previously discussed. I further agree to provide these accelerated amounts as the
contemplated periods of time arrive.
4. I absolutely and completely agree, promise and bind myself that I will never again
communicate, bother, go near or in any manner propose any amorous suggestions to
Journnie Marianne Escano. Any concerns regarding this undertaking and agreement
shall solely be coursed to Maria Fe Escano, the mother and guardian of Journnie
Marianne Escano.
IN WITNESS WHERFORE, I hereto affixed my signature this ______ day of
___________________ 2016 at Santa Cruz, Laguna.
