1. I D car d applicat ion shall be FREE OF COST only f or one t im e and af ter t hat f or
change of Permanent Address the applicant/ Reissuance of ID card/ for any reason shall be
chargeable @ Rs 300/- payable to TFI.
2. Application form should be filled in capital letters only. All informations are compulsory.
3. Sub-Jr. & Junior Player must enclose copy of Date of Birth certificate duly attested by
President/ Secretary of STA & DTA. DOB once entered shall not be changed.
4. No ID card application shall be taken into consideration by STA without the recommendation
and attestation of DOB certificate (for Sb-Jr., Cadet and Junior.) by President/ Secretary of
DTA.
5. All ID card application will come through club to DTA and DTA to STA.
