I Sudhir Vasudeo Joshi, residing at 5, Anand Niwas, 234 J S S Road, Mumbai – 400004 do
here by state that :

I am regularly filling my income tax returns under PAN No AACPJ0436D which was
allotted to me by income tax department.

The above mentioned pan number was deactivated by department.

I am regularly using above mentioned pan number I am not aware nor have I used any
other Pan number.
My Pan No. AACPJ0436D shall be active an any other pan number allotted to me shall
be cancelled.

I hereby indemnify the assessing officer for any loss occurred on cancellation of any
other pan number allotted to me.

Whatever stated above is true and correct to my knowledge and belief.
