To obtain a background check through Norred and Associates, follow these steps: [1] Fill out and sign the consent form and include a copy of your ID; [2] Include a $18 money order made payable to Norred and Associates; [3] Mail these items to Norred and Associates' background department. FOR OBTAINING BACKGROUND CHECK

1. Fill out completely and sign the attached consent form.

2. Make a copy of Georgia Drivers License, Georgia State issued I.D. Card or
passport and send with consent form.

3. Get a $18.00 money order and make payable to Norred & Associates.

4. Mail the items listed above - consent, money order and copy of ID to:

I hereby authorize Norred and Associates to request any present or former employer,
school, police department, financial institution, drug screening facility or other persons
having personal knowledge about me, to furnish bearer with any and all information,
including but not limited to criminal and driving history, in their possession regarding me
in connection with an application for employment. I am willing that a photocopy of this
authorization be accepted with the same authority as the original, and I specifically waive
any written notice from any present or former employer who may provide information
based upon this authorization request. I understand this authorization is to be part of the
written application that I sign.
This form serves as written notification that a Consumer report or Investigation Consumer
Report will be requested and used for the purpose of evaluating me for employment,
volunteer or associate in any manner or for promotion, reassignment, or retention as an
employee or volunteer.
