PETITION
The undersigned Petitioner unto this Honorable Court most
respectfully alleges that:

1. Petitioner Atty. Fernando L. Tanglao, Jr. (hereinafter referred


to as “Petitioner”) is a Filipino, thirty-five (35) years of age,
married and presently resides at Agawan St., Brgy.
Sohutan, Borongan City, Eastern Samar.

2. Petitioner is a resident of the Philippines for thirty-five (35)


years and presently maintains a regular place of work or
office at Tanglao Law Offices, Real St., Brgy. Songco,
Borongan City, Eastern Samar.

3. Petitioner was born on 09 September 1984 and his contact


numbers are (055) 560-0074 (landline) +63 966-228-0069
(mobile).

4. On 15 March 2011, Petitioner has completed his law studies


at Leyte Colleges College of Law, Tacloban City, Leyte.

5. On 22 March 2012, Petitioner has been admitted as a


member of the Philippine Bar with Roll No. 60002;

6. Petitioner was issued a Professional Tax Receipt No.


8578040 on January 17, 2020 at Borongan City and IBP
Membership Dues No. 110345 on January 16, 2020 for
Calendar Year 2020;

7. Herein Petitioner is a member of the Philippine Bar in good


standing with clearances attached herewith from the Office
of the Bar Confidant of the Supreme Court and the
Integrated bar of the Philippines;

8. Petitioner has no pending administrative case as shown in


the Certificate of the Bar Confidant and the Certificate from
the Integrated Bar of the Philippines.
9. Petitioner has not been convicted in the first instance of any
crime involving moral turpitude;

10. Petitioner undertakes to inform the Executive Judge of


this Honorable Court within ten (10) days if there is any
change in the office address as herein indicated; Petitioner
shall submit the monthly report as required by the rules on
notarial practices;

11. Petitioner has not applied for commission with any


other jurisdictions Petitioner was issued a Notarial
Commission No. 02-22-2018 valid until 12-31-2019 and is
now the subject for renewal;

12. Petitioner is currently holding a co-terminous position


in LGU – Borongan City as its City Legal Officer and is issued
by the City Mayor an Authority to Engage into Private
Practice of Law;

13. Petitioner is a holder of MCLE Certificate of


Compliance for the VI compliance period bearing numbers
VI – 0030014 dated January 16, 2020 valid until April 14,
2022;

14. Petitioner possesses all the qualifications and none of


the disqualifications of a notary public and has read,
understood and is willing to abide with the 2004 Rules on
Notarial Practice (A.M. No. 02-8-13-SC) which took effect on
August 1, 2004;

15. In support of this Petition, attached and made integral


parts hereof are the following documents:

a. MCLE Compliance Certificate No. VI – 0030014 (Annex


A)
b. Community Tax Certificate issued by the Treasurer of
Borongan City with numbers 24743870, dated January
16, 2020 (original) (Annex B);
c. Certificate of Membership in the Philippine Bar
(original) (Annex C);
d. CERTIFICATIONS of Good Moral / Good Standing
i. Certificate of Good Standing from IBP Main Office
(original) (Annex D);
ii. Certificate of good moral character / good
standing from IBP Eastern Samar Chapter
(original) (Annex E);
iii. Certificate of Good Standing/ Good Moral
Character from the Office of the Bar Confidant
(original copy) (Annex F);
e. Professional Tax Receipt for the year 2020 with Nos.
110345 on January 16, 2020 for Calendar Year 2020
(Borongan City) PTR (photocopy) (Annex G);
f. IBP Official Receipt No. 110345 dated January 16,
2020 for Calendar Year 2020 and issued by IBP
National Office for the Account of Eastern Samar
Chapter (Original) (Annex H);
g. Three (3) sets of specimen signature (Annex I);
h. Three (3) sets of specimen dry seal (Annex J);
i. Three (3) pieces of passport-size color photographs
with signatures taken within thirty (30) days of this
application (Annex K);
j. Receipt of Payment of Filing Fee of this Petition
(Annex L);
k. Memorandum of Authority to Engage into Private
Practice of Law; (Annex M).

PRAYER

WHEREFORE, it is respectfully prayed of this Honorable


Court that an Order be issued renewing the commission of the
Petitioner as a Notary Public of Borongan City, Eastern Samar.
Petitioner prays for such other reliefs as may be just and
equitable under the premises.

I, FERNANDO L. TANGLAO, JR., of legal age, Filipino citizen,
married and with postal address at Real St., Songco, Borongan City,
Eastern Samar, after having been duly sworn in accordance with law,
hereby depose and state that:

1. I am the Petitioner in this above-entitled Petition;


2. I have caused the preparation of the same;
3. I have read the contents thereof and that the allegations
therein, as well as the annexes thereto attached, are based
on my own knowledge and belief, true and correct and are
based on authentic records in our possession;
4. I have not heretofore commenced any action or filed any
claim involving the same issued in any court, tribunal or
quasi-judicial agency and, to the best of my knowledge, no
such other action or claim is pending therein; and
5. Should I thereafter learn that the same or similar action or
claim has been filed or is pending, I shall report that fact
within five (5) days therefrom to this Honorable Court.
