Comme. / Assistant Commissioner (CBS). (1) Application as per Annexure “A’ (Proforma enclosed) (2) Attestation form as per Annexure “B” (Proforma enclosed) (3) Copy of appointment letter of employee in the Customs Brokers firm / company: Tor fesh card as well renewal card arising out of ehange in employment (4 Proof of education qualification (10 + 2) duly attested (for / #1 Card) & Atiested copy of relevant degree certificate (for F Card) (6) Photo ID residential adaress proof of employee, duly attested (6) Police vetticetion pon in respeet of person for whom H Card #8 applied (7) Two recent size photograph of the employee in case of fresh card as well as renewal card (8) Copy of letter of employer / previous employer (Customs Brokers) having surrendered F/G fH Card, in case of renewal) (9) Copy of the Publie Notiee / Order vide which qualified for F/ G/M Card (10) Contact Phone / Mobile no. / E-mail of applicant cardholders 5, On receipt of the application, complete in all respects, Dy. / Asst Commissioner of Customs, CB Section shall scrutinize the request and issue of photo identity in form ‘F?, “GC” rH ay the case may he. The Photo identity card issue shall be valid for a period of five years for “H’ car from the date of issue or until it is surrendered, whichever is early and shall be renewed each time ‘ve years on the request of the employer (Customs Broker) well in for a further period of advance, prior to the expiry of validity. Photo identity card issued shall be valid till validity af licence in case of F/G card from the date of its issue or until it is surrendered, whichever venewal, there is no need to submit all the documents which were submitted at isearly.
