Complete dentures (CDs) - centric relation registration phases

1. Upper occlusion rim adjustment

Occlusal plane - the occlusal plane is defined as the average plane established by the incisal and occlusal surfaces of the teeth

Paralell in the anterior area of the Upper rim to the the interpupillary Line and in the posteriori area to the Camper’s Line

The vertical length – anteriorly extend 2mm below relaxed lip

The labial form of occlusal rim should provide adequate lip support.

2. Vertical dimension registration

Rest vertical dimension (RVD/VDR) - The distance between two selected points on maxilla and on the mandible when the
maxillofacial musculature is in a state of tonic equilibrium.

Occlusal vertical dimension (OVD/VDO) - The distance between two selected points when occluding teeth are in contact.

Free way space (interocclusal distance, FWS) - The distance between the occluding surfaces of the maxillary and mandibular teeth
when the mandible is in the physiologic rest position

RVD-FWS=OVD

In complete denture fabrication for edentulous patients the free way space should be 2-4 mm at premolar region.

Patient must sit in upright position and be relaxed - facial measurements (between reference points) after swallowing and relaxing,
some nice conversation.

Observation of lips contact can be helpful (Wierzyński method) – in RVD lips have only a linear contact, in OVD there is a surface
contact between them.

3. Lower occlusal rim adjustment

When the OVD is measured, then the Lower occlusal rim is modified to obtain the measured height.

4. Horizontal jaw relation registration

The relations of the mandible to maxilla in a horizontal plane or in anteroposterior direction.

Centric Relation - the most retruded relation of the mandible to the maxillae when the condyles are in the most posterior unstrained
position in the glenoid fossae from which lateral movement can be made, at any given degree of jaw separation

The centric relation is obtained when patient swallows or moves the tongue to the posterior part of the hard palate (mandible goes
after the tongue). There is also possibility of obtaining the centric relation by using manual manipulation and guidance.

5. Joining the rims at centric relation


6. Smile Line, Midline and Canine lines registration

Removable partial dentures (RPDs) – if patient has support on opposing teeth and the vertical
dimention of occlusion is preserved – we need to register the occlusion (either with interocclusal record or
occlusal rims depending on edentulous areas). When rims are used – the occlusion should look the same with
and without rims in the mouth.

If patient hasn’t got any remaning teeth contacts – we have to reconstruct the vertical dimension of occlusion,
so the phases looks like in edentulous patients.
