Waiver of Mtop Franchise

Lorna A. Añasco held a franchise for a motorized tricycle with permit number 1664 issued by the city mayor of Tagum City. Due to high maintenance costs, the tricycle ceased operating. Añasco waived all rights and interest in the franchise permit in favor of Rolando A. Calzada. Añasco executed the waiver/relinquishment of rights to transfer the franchise permit legally to Calzada. Calzada accepted the transfer of rights to the franchise permit.

I, LORNA A. AÑASCO, legal age, married, Filipino, and resident of Maharlika,


Magugpo East, Tagum City, Davao del Norte, represented by her attorney-in-fact, Mr.
Edilberto A. Calzada, by virtue of Special Power of Attorney denominated as Doc. No.
15; Page No. 3; Book No. XXII; Series of 2012 before Notary Public Atty. Israel E.
Torentera, after having been duly sworn to in accordance with law hereby depose and
say, THAT:

I am the franchise holder of a motorized Tricycle for Hire with the Motorized
Tricycle Operator’s Permit (MTOP) No. 1664 issued by the office of the City Mayor,
Tagum City;

The said Tricycle ceased to operate due to high cost of maintenance; thus, I
hereby WAIVE all my rights and interest over the franchise of the said MTOP No. 1664 in
favor of ROLANDO A. CALZADA, of legal age, Filipino, married and a resident of Prk. 3-B,
Apokon, Tagum City, Davao del Norte;

I am executing this waiver/relinquishment of rights to attest to the truth of the


foregoing facts and for whatever legal purpose it may serve.
