ARKY CONSTRUCTION AND SUPPLY, contractor of Replacement of Punong Bridge & App.
(From Temporary To Permanent) Along Sibuyan Circumferential Road in Sibuyan Island,
Romblon requesting for the issuance of Construction Work Pass in compliance to
Department Order No. 35, series of 2020.

Enclosed also is the list of the contractor’s staff/ workers with their respective ID pictures
assign in the above-mentioned project.
