EXPENSE CLAIM FOR MONTH OF AUGUST AND
SEPTEMBER 2019 AMOUNTING PHP 1,960.00

This is to respectfully justify the late submission of travel expense claim for the
month of August and September 2019 of the undersigned amounting Php 1,960.00. I
was supposed to submit the required documents on 1 st week of September but due
to the fast-tracking of documents for the 17.5 Million MOOE projects, I forgot to
submit the said documents. However, I will try to be prompt in future with any
concerns, which needs submission on time. Thus, the undersigned respectfully
request for the approval of the travel expenses claim for August and September
2019.
