AFFIDAVIT OF LOSS

I, Filipino, single, a permanent resident of ___ after having been duly


sworn to in accordance with law, hereby depose and say that:

1. Sometime in the year 2011, I was issued a Philippine Passport by the


Department of Foreign Affairs. However, I can no longer recall the Passport
Number of the said Passport;

2. On November 8, 2013, our house was flooded during the onslaught of super
typhoon Yolanda and to my knowledge my passport was lost during the said
typhoon;

3. Despite diligent search and efforts to locate the said passport, I could not find
the same such that I now believe that it is now lost beyond recovery;

4. This affidavit is being executed to attest the veracity of the foregoing facts and
to support the application for the issuance of a new one in lieu of the one
which was lost.
