AFFIDAVIT OF LOSS

I, EDUARDO M. LEJAO, Filipino citizen, single, of legal age, and a resident of Aritao,
Nueva Vizcaya, after having been duly sworn to in accordance with law, depose and say:

1) That I am the owner of a Philippine Passport issued by the Department of Foreign


Affairs - Tuguegarao City, the details of which are as follows:

Name of Passport Holder: EDUARDO M. LEJAO


Passport Number: VVO615961
Date Issued: August 6, 2007
Expiry Date: August 6, 2012;

2) That the said Passport was kept in my table drawer at home ever since it was issued
to me;

3) That when the time came for me to renew it in view of my impending travel abroad,
I noticed that my table drawer has no longer any contents;

4) That I tried to locate the said Passport but to no avail despite diligent efforts to
locate the same, and is now considered lost for all legal intents and purposes; and,

4) That I execute this affidavit to attest to the truthfulness of all the foregoing facts
and for the purpose of applying for the re-issuance of the said passport in my
name.

Affiant further says none.
